How screen readers actually move through a page

Dynamic content is invisible unless you say otherwise
Modern websites update without page reloads. Screen readers don’t notice unless the change is announced.

Third-party widgets inherit your liability

If it’s embedded, it’s your problem.

Automated testing can’t validate screen reader experience
